of bag/material you will be sealing (list all), product being packaged,

Vertrod or Technaseal Sealer Model#, Serial # and the Part# (if
known). If part# is not known, please provide a detailed description of
the part with measurements and any identifying numbers.
For TECHNICAL SUPPORT questions, please include Model#, Serial#
